Grade 3-8 Evaluation Form (click to download form)

• Each Head AND Assistant coach should complete form separately and submit to admin@wellesleysoccer.org.  Deadline for grade 3-8 coaches is Friday Nov. 15th.

• Please note CHANGES to the evaluation forms for the Fall 2024 season.

  ** ALL ROSTERED PLAYERS need to be ranked as field players. 

  ** Coaches WILL rank their own child.

  There are 2 tabs in the attached spreadsheet.  Please use the worksheet tab to help with your rankings. You can also refer to the Player Development Matrix to help quantify your evaluations. 

  Please complete Column A in Rank tab – rank ALL of your players by skill level/impact. No duplicate rank or ties.  Make sure you comment on all players, so that this can be used for team selection discussions. 

   There is a player commitment section (column E and F) for game attendance and % of practices attended (rough estimates). We need to know this in order to assess if player should be taking a spot on a stratified team and/or how the commitment level across the entire team impacts the team size you need to carry. 

  Please note a separate section to rank any players who regularly plays goalkeeper. 

  Each Head and Assistant coach should do their own evaluation form separately (and submit separately).

• Rank order your players with no duplicates into groups A, B, C and within those groups, please rank 1,2,3,4.
   
• Coaches should include rank for their own child
